Special Meal Accommodation
Garden Grove Unified School District provides modified menus for students who have a disability, medical condition, or special dietary need and are unable to consume a regular breakfast, lunch or snack. Our policy is under a federal mandate to include all students in school nutrition programs (Child Nutrition and Food Distribution Management Bulletin, No. 07-2014, California Department of Education)
Meal Substitutions for Students with Disabilities or a Medical Condition
USDA Regulation 7 CFR Part 15b requires substitutions or modifications in school meals for children whose disabilities restrict their diets. A child with a disability must be provided substitutions in foods when that need is supported by a signed statement from a recognized medical authority (licensed physician, physician assistant, or nurse practitioner).
Medical Statement for Children with Special Dietary Needs
Each special meal accommodation must be supported by a statement explaining the requested food substitution(s) and must be signed by a state-licensed healthcare professional (i.e. licensed physician, physician assistant, or nurse practitioner). The medical statement form must be completed in its entirety.
Please return all completed forms to the nurse or health assistant at the child’s school. The form will be sent to the district food service office for processing.
Special meal accommodations may take up to two weeks to process.
